Patients discharging from higher levels of care are given intake priority to minimize the likelihood of relapse as they return home. To facilitate a seamless transition, please send us a discharge summary at your earliest convenience. We are also happy to provide you with follow-up reports if you are collecting this data.
If you are referring your patient to our IOP for complementary clinical support of your own work, we want to collaborate closely with you. As standard operating procedure for IOP, patients are required to remain in individual psychotherapy concurrently with IOP. Throughout their participation, we fax a biweekly Team Update to all members of the patient’s health-care team and appreciate team members’ input to our work with mutual patients. When your patient has completed IOP, a copy of his/her Relapse Prevention Plan will be sent to you, along with a final Team Update. If you require different levels or types of collaborative efforts, you will find us flexible and cooperative!
