Prior to your first appointment, please click on Intake Paperwork to print and complete the documents that apply to you.
If we will be billing your insurance company, bring your insurance card to your first appointment. We will need to photocopy both sides of the card. For more information about insurance coverage, click on Fees & Insurance.
Finally, please remember that payment in full (the full fee if you are not covered by insurance or your copay or coinsurance if you are covered) is due at time of service. We do not accept credit or debit cards.
You may pay in cash or by check made out to Brenda L Wolfe PHD PC.
